Grey Bruce has submitted its Ontario Health Team Application to the Ministry of Health.
A release from the Grey Bruce Ontario Health Team Planning Committee says the application is supported by signatures from 20 health service provider boards and organizations across Grey and Bruce counties.
The Government of Ontario mandated changes to the healthcare system with an aim to build a more connected, sustainable healthcare system centered on
the needs of patients.
The Province believes the key is to create Ontario Health Teams (OHTs) where health care providers work as a coordinated team to make it easier for patients
and communities to navigate the system and transitions between healthcare providers.
The Grey Bruce Ontario Health Team (GB OHT) says its partners cover a broad scope of services including primary and acute care, rehabilitation, complex continuing care, mental health, long-term care, and support services.
The application to the Ministry of Health outlines the local approach to providing integrated care and redesigning service delivery.
The Health Team says stakeholders have been identified for phased-in consultation for the first year’s planning objectives, they are mental health
and addictions and transitions in care for frail seniors.
