The Grey Bruce Ontario Health Team is looking for input from the community as it works to assess healthcare in Grey Bruce.
Patients, clients, residents and caregivers are being asked to share their thoughts and experiences in a survey to help the coalition of organizations better support the community where it’s needed.
The formation of the Ontario Health Team model has been mandated by the Provincial Government as the new method of streamlining healthcare services. These Health Teams are to be made up of local care providers, and are meant to ensure people get the care they need through partnerships between local services.
The idea is to be able meet the unique needs of a community while also getting rid of duplication of services.
The Grey Bruce Ontario Health Team says on its website, “This change is intended to ensure all care, including primary care, hospital services, mental health and addiction services, long-term care and home and community care, will be easily accessible with seamless transitions between healthcare providers.”
The survey asks questions about which parts of the system have worked and which need improvement.
The Health Team asks asks what you think of its focus for its first year of work, which will be improving care transitions for frail seniors and also supporting patients living with Mental Health and Addictions issues.
