The Transportation Safety Board of Canada (TSB) has released its investigation report into a fatal plane crash in Orillia last summer.
In August, an amphibious Cessna, carrying six family members and the family dog, took off from the Orillia Rama Regional Airport to a cabin on Upper Raft Lake.
However, according to the TSB, the aircraft touched down on the water of Upper Raft Lake with the wheels in the down position.
As a result, the plane flipped over, coming to rest in an inverted position.
The pilot, Jeff Mavor, was trapped in the aircraft along with the family dog, who both died, but his wife and five children managed to escape.
The TSB report concludes weather was not considered a factor.
It says the aircraft had been in a previous accident in 2013, but it had been completely rebuilt by the pilot, who was also a licenced aircraft maintenance engineer.
